By analyzing real-world treatment patterns, RWE provides deeper insights into the effectiveness of osteoporosis treatments. Unlike clinical trials, which operate under controlled conditions, RWE captures the realities of routine clinical practice. &nbsp;</p><p>Worldwide, osteoporosis is estimated to cause fractures in a staggering one in three women over the age of 50, and in women over 45 years of age, osteoporosis accounts for more days in hospital than many other diseases, including diabetes, myocardial infarction and breast cancer.</p><p>With 71% of patients who sustain fractures related to osteoporosis remaining undiagnosed and untreated, data around treatment journeys is essential to help close the gap. A comprehensive picture of how patients experience care, manage their condition and navigate healthcare systems is lacking.</p><p><em>Highlighting the impact of RWE at WCO-IOF-ESCEO</em></p><p>At the 25th WCO-IOF-ESCEO Congress in Rome, Italy, ֳ, its partner, Amgen and investigators from supported investigator-initiated studies, are presenting eight abstracts, further building the body of RWE that can be used to elevate care for those at high risk of fracture. &nbsp;</p><p>These data will allow us to better characterize patients at high fracture risk and get a clearer picture of their treatment journeys. These insights are guiding how we support clinicians to prioritize early, targeted interventions for patients most at risk of fracture. &nbsp;</p><p>At ֳ, it is our mission that everyone with osteoporosis at very high fracture risk (VHFR) receives the appropriate investigation and intervention that they need, with the aim to create a world free of fragility fractures.</p><p>We are fully embracing the opportunity that RWE brings, generating data that deepens understanding of VHFR care and informs better treatment decisions. Around the world, the treatment gap in osteoporosis continues to be as pervasive as it is affecting; in an international survey last year, the International Osteoporosis Foundation (IOF) revealed that almost one in two women after the age of 50 and over experienced a fracture. Paradoxically, bone health – and the chance to identify and treat the underlying osteoporosis – was neglected during medical check-ups. The ramifications of this missed opportunity are huge.</p><p>Why does this matter? The impact on daily life for women with osteoporosis should not be underestimated, with fractures marked by excruciating pain, immobilization, and prolonged hospitalization, robbing them of their independence. This can have a significant impact on their well-being, leaving women feeling anxious and isolated. The lack of appropriate intervention at this crucial trigger point of the patient journey places a staggering burden on healthcare systems, with the cost of fractures in the EU anticipated to hit 77 billion EUR by 2050. &nbsp;</p><p>In the past year, we have made great progress in elevating secondary fracture prevention on government agendas – for example, <a href="https://pharmaphorum.com/patients/how-post-fracture-care-programmes-such-fracture-liaison-services-can-bridge-treatment-gap" target="_blank">the success in Japan following its reimbursement scheme for Fracture Liaison Services (FLSs)</a> and last year’s announcement of a <a href="https://www.osteoporos.org/ja-till-nytt-kunskapscentrum-for-osteoporos-i-stockholm/#:~:text=P%C3%A5%20ett%20m%C3%B6te%20i%20H%C3%A4lso,som%20f%C3%B6rebyggande%20%C3%A5tg%C3%A4rd%20mot%20sjukdomen." target="_blank">government-funded osteoporosis knowledge center in Stockholm, Sweden</a> – however, global efforts to tackle the treatment gap fall short of what is truly necessary. And, with the global population aging at a phenomenal rate, it is rapidly becoming a challenge we can no longer afford to ignore.</p><p><em>Closing the gaps in osteoporosis</em></p><p>As global leaders of the bone health community gather in London this week for the World Congress on Osteoporosis, Osteoarthritis and Musculoskeletal Diseases, we have a unique opportunity to confront some of the barriers that prevent us from the delivery of optimal patient care:<br><br><strong>Knowledge gap</strong>: IOF’s survey highlighted a disconnect between the occurrence of fractures amongst women at high-risk of osteoporosis and the decision by healthcare professionals to perform a diagnostic scan. And, as described by Kassim Javaid in the latest episode of the ֳ podcast, Fragility Fractures in Focus, the lack of a recognized ‘bone specialism’ has resulted in a lack of clarity around which practitioners ‘own’ this area of medicine, and consequently, how it should be delivered effectively. How can we therefore equip ALL healthcare practitioners with the knowledge and the tools they need to ensure everyone with osteoporosis receives appropriate investigation and intervention after experiencing a fracture?<br><br><strong>Fragmented pathways</strong>: In most healthcare systems, patients who experience a fracture are often faced with disjointed pathways that are unable to deliver the necessary assessment for osteoporosis, once again, leading to missed opportunities for timely diagnosis, initiation of appropriate treatments and implementation of preventive measures to reduce the risk of subsequent fractures.</p><p><strong>Patient education and empowerment</strong>: Empowering patients with accurate information, clear guidance in treatment regimens and supporting resources can enable them to make informed decisions and take an active role in their care by having effective conversations with their doctors.</p><p><em>Post-fracture care – make or break?</em></p><p>Fortunately, a means to bridge these gaps already exists. Coordinated, multidisciplinary services such as FLSs enable rapid referral, facilitate early intervention and improve secondary fracture prevention. These services provide a seamless pathway post-fracture by systematically identifying patients, assessing their fall and fracture risk, and carefully evaluating the most effective intervention, placing the individual’s needs at the center of care. FLSs have been shown to be both clinically and cost-effective , and are described as the single most important step in directly improving patient care and reducing spiraling fracture-related healthcare costs worldwide.</p><p>But having a solution is only half the challenge. Global implementation of coordinated post-fracture services is dependent on the close collaboration between policymakers, payers, healthcare professionals and the pharmaceutical industry, and a unified commitment to elevate secondary fracture prevention as a healthcare priority.</p><p>We are proud to work closely with partners such as the Fragility Fracture Network and IOF, which look to make this vision a reality – IOF’s <a href="https://www.capturethefracture.org/" target="_blank">Capture the Fracture®</a> program recently celebrated a tremendous milestone of 900 registered FLSs worldwide, doubling the number of services since 2019. &nbsp;To support those looking to start a new FLS or improve the performance of an existing service, we and our partners at Amgen established the FLS Academy and Network (FAN), an educational program led by a Scientific Steering Committee of international experts. FAN offers healthcare professionals practical resources, expert guidance and access to knowledge-sharing events designed to help overcome the challenges faced by those on the ground.</p><p>The evidence is in front of us – we already have the tools at our disposal to close the treatment gap in osteoporosis. Despite taking the virtual stage, ASBMR remains to be a prominent centre point for the latest clinical and scientific developments in bone health. That is why we are excited to be taking part in this year’s congress, connecting with the osteoporosis community and hearing about the latest advances in bone health from around the world.<br><br>We believe that ongoing research is essential to keep the field moving toward optimal osteoporosis management and fracture prevention, but how much progress has been made with recent innovation in osteoporosis? A few years ago, we saw a welcome shift in the way osteoporosis is managed with the implementation of the Fracture Liaison Service (FLS). Best practices like the FLS have already undergone rigorous improvement over time, and their benefit is already widely acknowledged. More uptake of these services will provide solutions to the disconnect between patient and healthcare professional. <br><br>In addition to best practice processes like an FLS program, patients and physicians alike could benefit from a more individualized approach to treating and monitoring osteoporosis and fracture risk. In recent years, working groups and expert consensuses have expressed the potential benefit of a treat-to-target strategy to reduce the risk of fragility fractures. We can learn from other therapeutic areas which have benefited from a disease-specific treat-to-target approach to individualise the treatment of each patient based on their probability of reaching a treatment goal. These groups have commented that progress toward reaching the patient’s goal would include periodic and systematic assessment.<br><br>For physicians and their patients with osteoporosis, this could open up a new and refined approach to use existing therapies. Ideally, using biomarkers that are reliable, easily measured and cost effective and setting targets that are realistic, adaptable and have evidence of the benefits, could result in a strategy to help reduce fragility fracture risk while providing important near-term feedback to patients that their treatment is making a difference.<br><br>ASBMR will see the sharing of the latest clinical and scientific developments in bone health but we need health systems and policy makers involved in bringing these innovations to life, ensuring osteoporosis is no longer overlooked and under prioritised. However, for innovative treatments to have maximum impact on patient care, they must be readily available to those who need them most. </div><div id="ext-gen975"><br></div><div id="ext-gen941">In the third of a series of thought leader articles by ֳ personnel, in partnership with the <a href="https://www.healtheuropa.eu/adoption-of-innovation-is-essential-to-the-future-of-osteoporosis-care/99167/" class="disclaimer-1" target="_blank">Health Europa journal</a>, we look at the adoption of innovation in osteoporosis care and management. Our goal is to ensure that the needs of the growing number of people at risk of fractures are highlighted. </div><div><br></div><div id="ext-gen978">Consider the fact: almost half of patients who suffer an osteoporotic hip fracture have experienced a previous osteoporotic fracture, but most of these patients did not receive treatment that could have reduced their risk of hip fracture. This represents a significant missed opportunity to prevent painful broken bones that can impact quality of life. </div><div><br></div><div>Innovation should focus on early diagnosis and prevention of future fractures; too many patients remain undiagnosed and untreated today, disconnected from the healthcare professionals who can help manage their disease, meaning they remain at risk of fractures that can lead to disability, loss of quality of life, and premature death.</div><div><br></div><div>The risk of fragility fractures rises with age, making osteoporosis care a priority for the future. As populations age, it will become increasingly important to ensure that older people have access to the services and care they need to stay healthy.</div><div><br></div><div>Some healthcare professionals are concerned by the level of access to innovation in osteoporosis. Research conducted by ֳ, showed that over a quarter (28%) of bone specialists rated the level of innovation in osteoporosis as below average or poor and, they agreed that health systems are not doing enough to support. If we continue on this path, millions of people risk missing out on an acceptable quality of later life as they age. Ageing is inevitable, but how we age is not.</div><div><br></div><div>Of course, innovation is not all about new therapies or technologies. Innovative models of service delivery play a vital role in fracture prevention. A few years ago, we saw a welcome shift in the way osteoporosis is managed with the implementation of the Fracture Liaison Service (FLS). Best practices like the FLS have already undergone rigorous improvement over time, and their benefit is already widely acknowledged. The availability of reliable biomarkers also promises to make fracture prevention more effective and personal. </div><div><br></div><div>Looking ahead, using artificial intelligence (AI) algorithms to analyse bone scans could help to identify patients susceptible to future fractures and lead to more effective clinical intervention, ultimately driving towards reducing the co-morbidities associated with osteoporosis. &nbsp;</div><div><br></div><div>For our part, ֳ is working to advance osteoporosis management and fracture prevention. We will continue to strive to deliver value to patients by developing new therapies and embracing the latest technologies.
